A prominent children’s hospital in Pittsburgh is under scrutiny as part of a broader federal investigation into potential fraudulent billing practices related to transgender treatments for minors. The UPMC Children’s Hospital of Pittsburgh has reportedly been identified in court filings by the U.S. Department of Justice (DOJ) as a facility where healthcare fraud may have occurred in connection with pediatric gender-affirming care.
Federal authorities stated they have evidence of fraudulent practices involving insurance billing tied to off-label prescriptions used in gender transition services. The information comes from an October 2 court filing, first reported by the Pittsburgh Post-Gazette.
Alleged Fraudulent Diagnoses to Secure Coverage
The DOJ filing alleges that some hospitals, including UPMC and Children’s Hospital of Philadelphia (CHOP), may have misrepresented children’s diagnoses to secure insurance coverage for puberty blockers and hormone therapies — drugs often prescribed off-label for gender transition in minors.
Though the DOJ notes that more direct evidence pertains to CHOP, investigators confirmed they are aware of concerning practices at UPMC as well. “There are allegations and evidence that raise concern that federal health care offenses may be occurring,” the filing states.
The DOJ’s broader investigation — part of President Donald Trump’s intensified review of transgender-related services for minors — is examining potential violations that include false statements and misuse of federal funds.
UPMC Quietly Ends Key Services Amid Scrutiny
UPMC has already discontinued certain gender-affirming services, including puberty blockers and hormone therapies for minors, citing the potential for prosecution according to the report. The hospital’s website currently states that gender-affirming surgeries are only provided to individuals aged 19 and older.
The decision to curtail services appears to coincide with the Trump administration’s legal position toward pediatric gender transitions.
In July, the DOJ subpoenaed multiple doctors and clinics involved in transgender medical care for minors, including UPMC, according to CBS News. The subpoenas seek documentation related to treatments, diagnoses, insurance billing, and internal communications.
A Nationwide Crackdown on Transgender Medicine for Children
The probe into UPMC is part of a larger federal campaign under the Trump administration to halt the use of taxpayer funds for what officials describe as “child mutilation” and “chemical castration.”
Earlier this year, Stephen Miller, White House Deputy Chief of Staff, declared that hospitals will no longer be permitted to use federal dollars for transgender surgeries or hormonal treatments on children.
“This includes the administration’s message to our hospital systems that they cannot and will not be allowed to use taxpayer dollars to perform chemical castrations and sexual mutilations of children,” Miller said in a May press briefing.
Federal officials from the FBI have also urged citizens and healthcare workers to report any medical providers engaged in these practices on minors, describing such treatments as a potential violation of federal law.
Medical Ethics Debate Intensifies
The DOJ’s investigation follows years of escalating political and legal controversy around pediatric gender transition. In 2024, watchdog group Do No Harm reported that American hospitals made an estimated $120 million over four years performing gender-transition procedures and treatments on nearly 14,000 children.
That same year, Dr. Eithan Haim, a surgeon and whistleblower from Texas Children’s Hospital, testified before Congress that transgender interventions on minors were destroying children’s lives. His testimony, and similar accounts from others in the medical field, have fueled growing calls for oversight and reform.
Critics of gender-affirming care argue the practice amounts to medical experimentation on vulnerable children, while supporters contend such care is essential for youth experiencing gender dysphoria. The Trump administration, however, has made its stance clear — and legal action may soon follow.
What’s Next for UPMC and Other Hospitals?
While the DOJ’s filings stop short of formally accusing UPMC of criminal conduct, the implication that “fraudulent billing” may have taken place — particularly involving federal healthcare programs — puts the institution at the center of a high-stakes legal and political storm.
With subpoenas issued and evidence under review, UPMC and other hospitals involved in transgender pediatric care could face civil penalties or federal charges if wrongdoing is confirmed.
